Key Responsibilities

Client Outreach and Engagement: Conduct outreach activities to engage individuals in the community, particularly those who are underserved or at risk, and encourage them to utilize mobile health unit services.
Assessment and Navigation: Assess clients' health and social needs, provide tailored information, and navigate them through the healthcare system to ensure they receive needed services and support.
Care Coordination: Coordinate with healthcare providers, social workers, and other relevant professionals to create a comprehensive care plan for clients, ensuring continuity of care and follow-up.
Resource Referral: Identify and connect clients to appropriate community resources, including medical, behavioral health, housing, and social services, to address their holistic needs.
Education and Advocacy: Educate clients about their health conditions, treatment options, and the importance of adherence to care plans; advocate on their behalf to remove barriers to care.
Data Collection and Documentation: Collect and maintain accurate and confidential client data, and produce reports as required for program evaluation and improvement.
Community Collaboration: Build and maintain relationships with community organizations, stakeholders, and other healthcare providers to enhance service delivery and client outcomes.

Required Qualifications

Education: Bachelor's degree in social work, public health, nursing, or a related field. Equivalent experience may be considered.
Will strongly consider individuals with Behavioral Health Tech (BHT) or Mental Health Associate (MHA) experience and /or those with previous experience working with the mental health/psychiatric population
Experience: Minimum of 2 years of experience in healthcare navigation, case management, or a related role. Experience working with diverse populations and knowledge of community resources is highly desirable.
Skills: Strong interpersonal and communication skills; ability to work independently and as part of a team; proficiency in Microsoft Office and electronic health record systems.
Language: Proficiency in English; bilingual skills in Spanish or other languages commonly spoken in the community are a plus.
Valid driver’s license with a clean driving record
